Or complete the On Line form HERE

If you download the form, once you complete it, bring it along with your dues to the next event, or scan the completed form and send to membership@lvmoc.net, or snail mail to the address below.

We accept PayPal payments, but require your contact information as described on the application form (you may fill out the online form or send that information to membership@lvmoc.net).  We also ask that you please add $1.00 when using PayPal and address the payment to treasurer@lvmoc.net .
